A lengthy and remarkably detailed medical letter documenting a patient’s prolonged struggle with severe urological illness. Writing to his physician in Philadelphia, Julius Wilcox provides a systematic account of symptoms including bladder pain, mucous discharge, urinary irregularities, and nocturnal emissions, along with observations on treatments prescribed (powders, warm-water injections, and bathing regimens). The letter is notable not only for its clinical specificity—rare in lay correspondence—but also for its emotional candor. Wilcox expresses anxiety, melancholy, and fear of “a miserable…end or insanity,” offering a striking window into the psychological burden of chronic illness in the mid-19th century. He also reflects on marriage, physical decline, and the contrast between outward appearance and internal suffering. Addressed to Dr. Husmann, 98 South 11th Street, Philadelphia, the letter reflects the period’s reliance on long-distance medical consultation and individualized treatment plans. Condition: Very good; folded as mailed, moderate toning and handling wear, but fully legible. Significance: An exceptional primary document for the history of medicine, particularly urology and patient experience, combining clinical detail with rare personal introspection.

Bladder trouble Dear Sir It is now nearly four weeks since I received your last, at which time you wished to hear from me again. I have taken the medicine as you directed, and a cold bath the other day on going to bed. Is it as good as any I think? The warm water injection I have used often for the last year. My present symptoms are the following. Pain in my back, legs, over the bladder and back of the scrotum. I think I may say that on the whole my pains have been less severe, for a few days than usual. It is at times very severe in my bladder though the urine is much as it has been for months. There is always a deposit of mucous in it, sometimes quite small and then large. Sometimes it keeps clear, and then it will turn turbid in an hour or so less perhaps. In quantity it is seldom more than 18 oz and usually not more than 12 oz and of a light color. At stool that which passes from the urethra looks like the white of an egg and not whitish like before. The pain back of the scrotum sometimes it is white when it passes, most often though is at times severe, and makes me very nervous. I have eased it several times by sitting down in cold water. Sometimes I can feel a moving there, and then there is a prick- ling sensation as though it were asleep. But one thing troubles me more yet. Within three weeks I have had several emissions at night notwithstanding the relief given to the parts in marriage. I have been very prudent ever since I was married, and since your last letter I have been more careful. Sometimes these discharges have occurred as often as every other night. Of course I am poor and weak. Still all tell me I look better than when I came home. I do not have as much pain as when I saw you, but farther than that I do not know as I am any better. I took the # powders after taking the fourteenth powder as directed, but I feel no difference when I take the # powders as I am aware of. I have enough to last about a week yet. In that time I hope you will find time to send me some more medicine and advice, and I hope that sometime I shall stand before you a well man. If you can save me from a miserable miserable end or insanity I shall ever feel grateful to you and I trust to my maker for your encouragement. Let me say what my friends tell me. They say they should not guess I was sick from my looks. It does not encourage me, for I know I am sick with a dreadful sickness. I think you have at least helped me a little, and I wish you to still do for me what you can. I hope you will be able to write me within 6 or 8 days again. Do what you can for me and if I die I can do so feeling I have tried to live. Yours with much respect Julius Wilcox Warrenton N.C. P.S. I need not tell you that knowing what I do of myself I cannot help sometimes being melancholy, but I try to shake it off. Dr. Husmann 98 South 11th St Phila
